What Causes Fissure ?

An anal fissure is a small tear or crack in the lining of the anus. It is a common condition and can be quite painful, causing discomfort during bowel movements. Anal fissures are typically caused by passing large or hard stools, often associated with constipation. However, they can also be a result of other factors such as diarrhea, inflammatory bowel disease, or trauma to the anal area.

Symptoms of an anal fissure may include:

  • Intense pain or discomfort during and after bowel movements.
  • Bright red blood on the stool or on the toilet paper.
  • Itching and irritation around the anus.
  • A visible tear or crack in the skin around the anus.
  • The pain caused by an anal fissure can be severe and may persist for some time after bowel movements,
  • leading to a fear of using the toilet and exacerbating constipation.

Most anal fissures can be treated successfully with conservative measures, including:

  • Increasing fiber intake: Eating more fruits, vegetables, and whole grains can soften the stool, making bowel movements less painful and reducing the risk of further injury.
  • Stool softeners: Over-the-counter stool softeners can also help in softening the stool, easing bowel movements, and reducing strain on the anus.
  • Sitz baths: Soaking the anal area in warm water for about 15 minutes several times a day can help soothe the area and promote healing.
  • Topical ointments or creams: Over-the-counter creams containing analgesics or hydrocortisone can help reduce pain and inflammation.

In cases where conservative treatments do not provide relief or the fissure persists, medical intervention may be required. A doctor may prescribe stronger medications, such as topical nitroglycerin or calcium channel blockers, to promote healing and relax the anal sphincter. In rare cases where fissures do not respond to conservative or medical treatments, surgical intervention might be necessary to repair the fissure..
